When scheduling your appointment with us, please “Sign Up” and create an account with a password so you will be able to store information, fill out your intake forms and have access to your booking history. Please fill out the intake forms before arriving for your appointment. Intake forms can only be filled out while booking an appointment or from your booking confirmation email. New clients: please arrive 5-10 minutes early to find parking and for consultation with your therapist.
A valid credit card is required to schedule an appointment. Your card will NOT be charged and you do NOT have to pay with a credit card. We ask for this information only for no-shows and cancellations within 24 hours. If you do not wish to store your card but would still like to schedule, please contact Heather at 206-617-3454 and she will schedule your appointment for you. Payment will be taken before your appointment and your card will not be stored. If you miss the session or wish to cancel within 24 hours, payment for the treatment in full is due. Payments are accepted via credit and debit card, HSA card, Venmo, cash, and local check.
INSURANCE: We do not accept insurance as payment nor do we accept PIP cases, L&I claims or Health Insurance as payment. However, we can provide you with a Superbill receipt for you to provide to your insurance carrier for reimbursement directly to you. Please indicate if you would like one when filling out your intake forms and ask your therapist to provide one for you. Superbills are not available when paying for massage treatments with a Gift Certificate. Please also note that Superbills are not available for treatments with Gabriela. It’s the client’s responsibility to provide any documentation needed. We do not provide medical records nor communicate with any third parties, including attorneys.
GIFT CERTIFICATES are available for purchase at www.hawthornemassagesc.as.me. Gift certificates are refundable up to 30 days after purchase. They are transferable and do not expire. Charity or Auction certificates donated by HMSC have no monetary value and expire 90 days after issue. Gift certificates cannot be used when a Superbill is requested or with any other discount codes or coupons. Gift certificates are not valid for sessions with Atsuko Arai.

Please reschedule or cancel at least 24 hours before the beginning of your appointment, or you will be charged a cancellation fee of the full price of your scheduled appointment. By booking, you authorize Hawthorne Massage and Self Care to charge your credit card a no-show fee of the full treatment price should you miss your visit without 24 hours notification. Exceptions may be offered for some emergencies and sudden illness and rescheduling your appointment within 7 days of your original appointment date will avoid any cancellation fees.
Our treatment rooms include high efficiency HEPA air purifiers and one of our rooms has an air extraction fan which improves the air quality and circulation of the treatment space. Facial masks are no longer required but if you would prefer to wear one or would like for us to wear one, your request will be honored. Disinfection protocols are in place in all areas of our clinic.
IF YOU HAVE FLU OR COLD SYMPTOMS OF ANY KIND, PLEASE RESCHEDULE YOUR APPOINTMENT. IF YOU HAVE AN EMERGENCY WITHIN THE 24 HOUR CANCELLATION PERIOD, PLEASE TEXT OR CALL YOUR THERAPIST DIRECTLY TO RESCHEDULE YOUR APPOINTMENT (YOU WILL NOT BE ABLE TO DO THIS ONLINE) OR HEATHER AT 206-617-3454.
